Aliannah Hope "Ali" Simms is the daughter of Leah Messer and Corey Simms and the step-daughter of Jeremy Calvert and Miranda Simms. She has a twin-sister Aleeah Simms and a younger maternal half-sister Adalynn Calvert. Ali and Aleeah both have a half sister on their dads side named Remington Simms.
She was born with a disease called Titin muscular dystrophy, which is a very rare form of the disorder that has only ever been found in adults.

- Aliannah is the only one of all the babies on the Teen Mom franchise to have a rare genetic condition.
- Aliannah and her twin were both born six weeks premature and had to spend the first few weeks in the hospital because their lungs were not properly developed.
- She has seen the same doctor, a neurologist named Dr Tsao since she was at least two years old; it was Dr Tsao who was successful in finally being able to make a diagnosis for Aliannah after several years of suffering from muscular problems.
- She is highly at risk to life threatening illnesses, such as the coronovirus, which over the last four years has proven to be less life threatening since the pandemic first started.
- Despite having disability access, Ali prefers not to use her wheelchair because she wants to be treated normally like her sisters without feeling mollycoddled because of her condition.
- Ali has recently expressed her dream of becoming a writer, as she wants to write about her experiences about how horses have helped her through her muscular dystrophy.
- Ali gets into frequent disagreements with her mother and her sister about what to do and not do regarding her MD.
